You can schedule an appointment in two ways:

  • You can call us directly at 1-800-TEMPLE-MED (800-836-7536) between the hours of 8:00 a.m. and 6:00 p.m., Mondays – Fridays.
  • If you’d prefer us to contact you, please fill out the form below. This form is secure and our Access Center specialist will call you back within one business day to schedule your appointment.

Note: This appointment form should not be used for urgent medical issues or emergencies. Please call 911, your physician or go to the closest emergency room for care and treatment.
